This area contains forms and other information for clients and prospective clients.  Each area is arranged by service type, and the forms are ordered according to when they are needed.  Some forms include exercises that I may suggest or assign to you.  Not all forms will be relevant to you.  

Where there are forms to be completed and returned, please print them out and complete and return them to me in person, by mail, or scan and email them to me. If you email documents with sensitive information, I recommend sending them only on a secured network.  You may also encrypt the documents if you wish, and share with me the decryption information.
